Medical Coverage

Masterplan offers regular full-time employees and their families the opportunity to elect medical coverage. Depending on where you live, you will have the option to choose between a traditional Preferred Provider Option Plan (PPO), or a Health Maintenance Organization Plan (HMO).

If you do not submit your benefits paperwork for medical coverage by your enrollment date stated on your offer letter, your benefits will be limited.

Vision Coverage

Masterplan offers regular full-time employees and their families Vision coverage. Once medical coverage is elected, you are automatically enrolled in our Vision plan. Upon your hire, a brochure will be given to you explaining the details of the vision coverage.

Dental Coverage

Masterplan offers regular full-time employees and their families the opportunity to elect dental coverage. Depending on where you live, you will have the option to choose between a Preferred Provider Plan (DPPO), or a Dental Maintenance Organization Plan (DMO).

As with your medical coverage, if you do not submit your benefits paperwork for dental coverage by your enrollment date stated on your offer letter, your benefits will be limited.

Life, Accidental Death & Dismemberment and
Long Term Disability Coverage (No Cost To You)

Masterplan will provide the following benefits at no cost to you:

  • Life is 2x your Base Annual Earnings to a maximum of $300,000, lesser of $5,000 or 50% of employees Basic Life amount for spouse, and $2,000 for each dependent.
  • AD&D provides up to 100% of the AD&D benefit for loss of life or limb due to an accident.
  • Long Term Disability will replace 66.67% of your monthly income to a maximum monthly benefit of $10,000 in the event you become disabled.

Voluntary Accidental Death & Dismemberment

You will have the opportunity to select Voluntary AD&D insurance for yourself and your family. The cost of voluntary coverage is based on your age and the amount of coverage.
